Please do give us as much notice as you can for New or Replacement PC requests, ideally 3 days before the New or Replacement PC’s arrives.

1. User Personal Details

2. New User Account Settings

The below options will be used to create the user account on your system as well as to configure the user's email settings. Please note a unique password will be generated for the new account.

Desired Username *

Desired Email Address for the user *

Does the user need to be part of any distribution emails (info@/sales@)? If so, which? *

Does the user require any other email addresses to be linked to them? *

Does this user need access to any other user's mailbox? *

Do any other users need access to the new user's mailbox? *

Which machine is the user going to be using? *

Does the user need to work remotely? If so, what do they need access to? *

3. Permissions

The information in this section will determine the system access levels of the new user’s account.

What level of permission does the user need? Can the user replicate the permissions of another user? *

Does the new user require any new folders or shared drives setting up? *

Does the user require administrative rights on their machine? *

4. Software

In this section, please detail which software the new user needs to have available to them.

Does the new user require access to any specific line of business application? *

If Office 365, Does the user need to install Office software (i.e. Business Premium License)? *

Does the user require any specific non-standard (nonMicrosoft) software? *

5. Printers and Screens

The below will define which printers the user will have access to and at which level

Which printers does the user need access to? *

Do they need any limitations to their printing (e.g. only black and white)? *

How many screens does the user currently have / would like to have? *

Agreements

I agree to the additional cloud charges as required. I agree that the information I have provided is correct.

I agree that (if required) this user will be an addition to my Intech Fusion Agreement and therefore will incur additional monthly fees.
